Statement of Intent
- I understand that the Intuitive Support Program, Intuitive Support Sessions, Reach Your Goals with the Moon Program and Reach Your Goals with the Moon sessions are comprehensive processes that may involve all areas of my life including work, finances, health, relationships, education and recreation. I acknowledge that deciding how to handle these issues and implement my choices is exclusively my responsibility.
- As a client, I understand and agree that I am fully responsible for my well-being during the duration of my Intuitive Support Program, Intuitive Support Sessions, Reach Your Goals with the Moon Program and Reach Your Goals with the Moon sessions, including my choices and decisions. I recognize that Intuitive Support and Reach Your Goals with the Moon coaching is not psychotherapy, nor a substitute for counseling or other mental health care, and I will not use it in place of any form of professional therapy. Professional counseling referrals will be given if needed.
- If I am currently in psychological therapy or otherwise under the care of a mental health professional, I will have consulted with this person regarding the advisability of using Intuitive Support and Reach your Goals with the Moon coaching and this person is aware of my decision to proceed with this spiritual counseling and coaching method. In addition, I will also inform Dana da Ponte that I am currently in therapy or under the care of a mental health professional.
- I understand Intuitive Support and Reach Your Goals with the Moon coaching is not to be used in lieu of professional advice and that I will seek such guidance, as needed, for legal, medical, financial, business or other matters.
- I understand that information will be held confidential unless I state otherwise, in writing, except as required by law. I understand that certain topics may be anonymously shared for instruction and/or teaching purposes.
- I hereby release, waive, acquit and forever discharge Dana da Ponte, her agents, successors, assigns, personal representatives, executors, heirs and employees from every claim, suit action, demand or right to compensation for damages
- I may claim to have or that I may have arising out of actions, omissions, or commissions taken by myself or by Dana da Ponte as a result of the advice given by Dana da Ponte or otherwise resulting from the working relationship contemplated hereunder. I further declare and represent that no promise, inducement or agreement not herein expressed has been made to me to enter into this release. The release made pursuant to this paragraph shall bind my heirs, executors, personal representatives, successors, assigns, and agents.
